Hospital fuck up, from the sound of it nurses thought they were ok if they simply changed the needle tip on insulin pens from patient to patient instead of getting an entirely new apparatus. Off the cuff guess is lousy training / policies.

No real way to tell how many people shared a given pen. It is a "may have been exposed" more than a "were exposed", but in either case very incompetent. And bad.
